In order to expedite your first visit to our office, please print and complete the Health History and Report Release Forms. If you have been involved in an automobile accident which is a contributing factor to your need for chiropractic care, please also complete our Personal Injury Information Sheet.  Also available here is our Systems Survey Form. These forms can be completed electronically using Acrobat Reader.

Do you take my insurance?

A: When insurance is involved we collect your insurance information and support you to be reimbursed by your insurance company, by out of network coverage or through your medical savings plan. We do not accept assignment or payment directly from any insurance company and are not a provider on any insurance network.

Q: What if my insurance doesn’t cover chiropractic care?

A: We believe that excellent care is a basic human right and have established our cash prices in such a way as to make it reasonably affordable.

Q: Do you take walk-in appointments?

A: Each routine visit with the Doctor is scheduled for 15 minutes of face-time to ensure that all issues are addressed and thorough care is given.  Therefore, each visit is by appointment only.

Q: Can I expect to receive treatment on my first visit?

A: Yes! If your initial exam demonstrates the need for chiropractic care and wellness services, you can expect to receive care on your first visit.

Q: Is a chiropractor a real doctor?
A: In short, YES! Chiropractors must be board certified and are considered a primary portal of entry into the healthcare system. As such, they must be competent to give a thorough physical exam, correlating exam findings and working with other health care professionals in the best interest of each patient. Chiropractors receive extensive education in the areas of orthopedics, neurology, radiology, anatomy and physiology. Dr. Slippy is certified by the National Board of Chiropractic Examiners. So bring your lab results, MRI’s and X-rays and he’ll be happy to go over them with you and answer any questions you may have.
